A. MOTION TO APPROVE Amendment 001 to Contract JZ024-15-2025 between the Areawide Council on Aging of Broward County, Inc., and Broward County, increasing the Contract value by $300,000 for a new Contract value of $2,077,811, effective July 1, 2024 through June 30, 2025, to expand home and community-based services to eligible individuals with Alzheimer’s disease and other forms of dementia, and their caregivers; authorize the Mayor and Clerk to execute same; and authorize the County Administrator to take all necessary administrative and budgetary actions to implement the Contract as amended.
B. MOTION TO ADOPT budget Resolution to appropriate revenue in the amount of $300,000 for services to eligible individuals with Alzheimer’s disease and other forms of dementia, and their caregivers.

Why Action is Necessary
A. Broward County Board of County Commissioners (“Commission”) approval is required for agreements with external agencies.
B. Section 129.06(2)(d), Florida Statutes, requires a budget resolution to appropriate unanticipated revenue in the budget.
What Action Accomplishes
Motions A and B provide increased funding for services to eligible individuals with Alzheimer’s disease and other forms of dementia, and their caregivers.

Summary Explanation/Background
THE HUMAN SERVICES DEPARTMENT AND THE ELDERLY AND VETERANS SERVICES DIVISION RECOMMEND APPROVAL OF THE ABOVE ACTIONS.
These actions support the Commission’s value of "building partnerships and working collaboratively with others to meet shared objectives" and the goal of "delivering accessible human services that holistically address the whole person collaboratively and compassionately.” This Amendment increases funds for services to individuals with Alzheimer’s disease and other forms of dementia, and their caregivers, thus addressing the special needs of eligible individuals.
The Alzheimer’s Disease Initiative (“ADI”) was legislatively created 30 years ago with the goal of providing funds for the continuum of services to meet the changing needs of individuals with Alzheimer’s disease and other forms of dementia, as well as their families. The Florida Department of Elder Affairs develops policies and administers programs and services to carry out the statutory requirements for this program.
The Elderly and Veteran Services Division (“EVSD”) has served as the lead agency for aging services in Broward County since 1986. Through this Amendment, EVSD will expend these funds for the provision of home- and community-based services to additional ADI clients. This Amendment also provides for the addition of two new service categories - emergency home-delivered shelf-stable meals and material aid. Current ADI services include case aide, case management, in-home respite, mental health counseling and screening, consumable medical supplies, and emergency and extended care for caregivers. No outcomes have been established.
State ADI funds are distributed through the Areawide Council on Aging of Broward County, Inc. (“Council”). The Council is acting on behalf of the state of Florida.
This Amendment (Exhibit 1) is in the Council’s format and was reviewed and approved as to form by the County Attorney’s Office. The Council will sign last following the Commission’s approval.
