Summary Explanation/Background

THE HUMAN SERVICES DEPARTMENT AND THE ELDERLY AND VETERANS SERVICES DIVISION RECOMMEND APPROVAL OF THE ABOVE ACTIONS.

These actions support the Commission’s value of "building partnerships and working collaboratively with others to meet shared objectives” and the goal to “delivering accessible human services that holistically address the whole person collaboratively and compassionately.” By acquiring these state funds and partnering with the Areawide Council on Aging of Broward County Inc. ("Council"), EVSD can provide home and community based services to assist eligible seniors in coping with daily living demands and aging in their own homes.

Agreement No. JC023-15-2024 funds in-home, adult day care, medical supply, and emergency response system services to eligible seniors under the Community Care for the Elderly (“CCE”) Program. This First Amendment increases funding to expand services directed at preventing, postponing, or reducing their inappropriate or unnecessary placement in nursing homes and other institutions. The grant funding will be increased by $289,348, from $7,269,882 to $7,559,230, and Broward County required match increased by $32,150, from $807,765 to $839,915. The new total contract value is $8,399,145. EVSD administers the funds, provides case management and support services; and maintains vendor agreements with licensed home health and other community agencies for the provision of the services.

The state of Florida CCE funds are distributed through the Council. As such, the Council is acting on behalf of the state of Florida.

This Amendment (Exhibit 1) is in the Council’s format and was reviewed and approved as to form by the Office of the County Attorney. The Council will sign last following the Commission’s approval.
