Summary Explanation/Background

THE HUMAN SERVICES DEPARTMENT AND THE ELDERLY AND VETERANS SERVICES DIVISION RECOMMEND APPROVAL OF THE ABOVE MOTIONS.

 

This program supports the Commission value of “approaching human services collaboratively and compassionately with special emphasis on the most vulnerable" and the goals to: (i) “effectively advocate for and acquire municipal, state, and federal financial support to equitably address health and human services needs of the entire community through a truly coordinated system of care”; and (ii) “deliver evidence-based services to the public and connect customers and their family members to sustainable support, with special emphasis on financial support”. By acquiring these local funds and partnering with the Council, EVSD can provide community-based services that allow seniors to age in place in their own homes with their familial caregivers.

 

Retroactive approval is sought due to delays with obtaining necessary documentation from the funder to finalize the agreement. This funding allows EVSD to provide case management, health support, and adult day care services to seniors. Eligible functionally or cognitively impaired individuals, 60 years of age and over, receive appropriate elder care assistance to prevent, postpone, or reduce their placement in nursing homes and other institutions.

 

Each County Fiscal Year, the Council makes local match funding available for case management and adult day care services for eligible seniors without requiring their qualification for the state-funded Community Care for the Elderly (CCE) Program. Services are made available to eligible recipients after the Council determines their eligibility and authorizes services. The Council does not require EVSD to serve a specific number of seniors. The Council does, however, require EVSD to ensure there is no dual enrollment with either the CCE Program or the Statewide Medicaid Managed Care Long-Term Care Program. While no performance measures are set by the Council, EVSD utilizes CCE program performance measures to determine the success of the program. EVSD has met these goals on a yearly basis.

 

This Agreement was prepared by the Council in its format and has been reviewed and approved as to form by the Office of the County Attorney; the Council will sign last following approval by the Commission.
