Why Action is Necessary

Section 20.31 (b)(3) Broward County Administrative Code, requires that the Broward County Board of County Commissioners (“Commission”) approves all state and federal grant agreements.

This action authorizes the Commission to continue its partnership with Broward Behavioral Health Coalition Inc. and receive grant funding for the provision of mental health services for eligible seniors in Broward County.

Summary Explanation/Background

THE HUMAN SERVICES DEPARTMENT AND THE ELDERLY AND VETERANS SERVICES DIVISION RECOMMEND APPROVAL OF THE ABOVE ACTION.

 

This program supports the Commission’s value of "approaching human services collaboratively and compassionately, with special emphasis on the most vulnerable" and the goal to "deliver evidence-based services to the public and connect customers and their family members to sustainable support, with special emphasis on financial supports."  This program provides targeted mental health case management and peer support to the most vulnerable seniors in the community.  Services also include two evidence-based programs:  SSI/SSDI Outreach, Access and Recovery (SOAR) and Wellness Recovery Action Plan (WRAP).  These proven practices result in these seniors maintaining their health and recovery in the community rather than an institution.

 

Community-based mental health services are funded by the Florida Department of Children and Families (“DCF”) that contracts with Broward Behavioral Health Coalition Inc. (“BBHC”) as the managing entity for Broward County. Contract No. 34345-20 (“Contract”) between BBHC and Broward County facilitates the provision of community-based mental health services to eligible residents served by the Elderly and Veterans Services Division (“EVSD”). Under the Contract, EVSD provides multiple services, including screening, comprehensive assessment, case management, coordination of resources, and peer recovery support to eligible residents receiving mental health services. The State of Florida fiscal year 2021 Contract outcomes are being met. This First Amendment replaces the Funding Detail exhibit, the Local Match Plan exhibit, and extends the Contract for an additional one-year term in the amount of $333,076, effective from July 1, 2021 to June 30, 2022.

 

The target and outcomes are as follows: a) EVSD will serve 81 adults diagnosed with a severe and persistent mental illness and 35 additional with forensic involvement (total 116); b) 90% of adults diagnosed with a severe and persistent mental illness live in a stable housing environment, and 67% of adults in forensic involvement live in a stable housing environment; c) 95% of case managers will attend at least 95% of the South Florida Monthly Discharge or Treatment Plan Meetings at the South Florida State Hospital, as reported by State Hospital staff; d) 95% of case managers will attend at least 95% of the monthly concurrent reviews and follow through with 100% of the recommendations from the prior month; e) 95% of clients with a comprehensive service and/or treatment plan will attain at least one goal or objective per quarter; and f) 100% of persons receiving state-contracted service event records which have matching initial admission records will be in compliance with the current DCF pamphlet.

 

Upon execution of this First Amendment, EVSD anticipates receiving $333,076 in funding, which requires a cash match of $3,857.  There is also an anticipated additional and unsecured amount of $66,615 for uncompensated units, which BBHC, at its sole discretion and subject to availability of funds, may pay Broward County in who, in part, or not at all.

 

The First Amendment will be prepared by BBHC in its format and will be reviewed by the Office of the County Attorney. BBHC will sign last following Commission approval.
